• Monitor and manage anti-coagulation therapy through our Outpatient Anti-Coagulation Care Clinic
• Perform renal dose adjustments for patients with kidney impairment
• Offer admission medication counseling to ensure accurate home medication reconciliation
• Provide discharge medication counseling to help patients understand and manage their prescriptions after leaving the hospital
• Ensure continuation of a patient’s home medications while admitted
• Adjust medications based on therapeutic drug levels to maintain safe and effective dosing
• Recommend appropriate antibiotics and drug dosages based on infection type and patient condition
• Deliver specialized counseling for patients on blood thinners at the time of discharge
• Provide vaccine education and counseling during hospital stay
